If you are aged between 14 and 17 and live permanently in the Republic of Ireland, you can enrol a change of name by Deed Poll.
You must complete the Deed Poll form, and both parents must sign their consent. In some cases, a court order may be required if consent from either parent is not available.
Complete the Deed Poll form
Fill out the Deed Poll form in front of a witness who is at least 18 years old.
Enter your old name exactly as it appears on your birth certificate, including all middle names.
If you have changed your name before, you can use this as your old name and provide proof. For example, a previous Deed Poll.
Get your documents certified
Make photocopies of:
- your birth certificate
- photo ID for the parent who completed the Supplemental Affidavit
- any supporting documents. For example, a parent’s death certificate
Uncertified copies will not be accepted.
You will need to have your document copies certified by one of the following:
- An Garda Síochána
- a practising solicitor
- a Commissioner for Oaths
Complete the affidavits
The witness who saw the Deed Poll being signed must swear an Affidavit of Attesting Witness.
The parent providing photo ID for the application must also swear a Supplemental Affidavit.
The affidavits must be sworn in front of either:
- a practising solicitor
- or a Commissioner for Oaths

Stamp duty is a government tax on legal documents. The documents must be stamped as follows:
- Change of Name by Deed Poll document: €60
- Supplemental Affidavit: €20
Lodge your documents
Note: Minor applications cannot be sent by post unless a solicitor’s cover letter is enclosed. Without this, the application will not be processed and will be returned.
