Deed Poll for adults

If you are 18 or older and live permanently in the Republic of Ireland, you can enrol a change of name by Deed Poll.

If you live in Ireland but are not a citizen of the European Economic Area or the UK, you must get a Change of Name Licence before enrolling a Deed Poll. This licence is issued by Immigration Service Delivery.

  1. Complete the Deed Poll form

    Fill out the Deed Poll form in front of a witness who is at least 18 years old.

    Enter your old name exactly as it appears on your birth certificate, including all middle names.

    If you have changed your name before, you can use this as your old name and provide proof. For example, a previous Deed Poll or a copy of your marriage certificate.

  2. Get your documents certified

    Make photocopies of:

    • your birth certificate
    • your photo ID
    • any supporting documents. For example, your marriage certificate

    Uncertified copies will not be accepted.

    You will need to have your document copies certified by one of the following:

    • An Garda Síochána
    • a practising solicitor
    • a Commissioner for Oaths
  3. Complete the affidavit

    The witness who saw the Deed Poll being signed must swear an Affidavit of Attesting Witness.

    This affidavit must be sworn in front of either:

    • a practising solicitor
    • or a Commissioner for Oaths

  4. Pay the fee

    Stamp duty is a government tax on legal documents. The document must be stamped as follows:

    • Change of Name by Deed Poll document: €60

  5. Enrol the Deed Poll

    Enrolling your Deed Poll adds it to the public Deed Poll Register. You must complete a Request to Enrol form.

  6. Lodge your documents

    Once you have completed all the above steps, you should include all documents and a stamped self-addressed envelope. You can then lodge it in one of the following ways:

    • In person: You can book an appointment to lodge the documents at the Central Office of the High Court. (Add link)
    • By post: Central Office of the High Court, Four Courts, Inns Quay, Dublin 7.
    • By DX (for solicitors only): DX 276001 Gandon Building.
